Registered Nurse – Case Manager
Location: 2401 W Belvedere Ave, Baltimore, MD 21215
Shift: Day | 5x8-Hour (8:00 AM – 4:30 PM)
Position Overview:
Seeking an experienced RN Case Manager to join a fast-paced acute care environment. The ideal candidate will provide comprehensive discharge planning, care coordination, and resource management across diverse patient populations.
Key Details:
- Schedule: Monday – Friday, 8:00a–4:30p
- Weekend Requirement: 2 weekend day shifts per month (schedule runs 1st–end of each month)
- On-Call: None
- First-Time Travelers: Not accepted
- Experience Required: Minimum 2 years of acute care case management experience
Patient Populations:
- Trauma
- Neurology
- Cardiac
- Medical/Surgical
- Open Heart
- Behavioral Health
- Bone Lengthening
Requirements:
- Licensure: Active MD RN license or Compact RN license
- Certifications: BLS (AHA) – required
- Experience: Strong background in acute inpatient case management, interdisciplinary collaboration, and discharge planning
Additional Information:
- Setting: Acute Care Hospital
- Floating: Within case management units as needed
