Description

$5,000 SIGN ON BONUS. The Physical Therapist is responsible for the provision of Physical Therapy interventions for each client as assigned. These interventions will be designed to enhance the physical capabilities of all assigned clients.

Requirements

Provide therapy sessions as scheduled for each assigned client.
Supply written progress notes after each scheduled session.
Submit monthly clinical reports for each client by the end of the week of that client’s Clinical Review.

Communicate as necessary with clinical team and residential staff regarding specific client issues.
Complete assessments on prospective and/or new clients as indicated.
As part of the Clinical Team participate in development of predicted outcomes for assigned clients.
Attend Clinical Team meetings and family meetings as indicated.
